I understand I am in no way required to participate with Riverside Submission/Arrey Jiu-Jitsu. 

I understand that I must sign the release of liability if I would like to participate in any of the named events. 

I understand that Riverside Submission/Arrey Jiu-Jitsu is permitted by law to require me to sign this release of liability before permitting me to participate in events or training sessions. 

I understand by signing this document I am forever agreeing to indemnify and hold Riverside Submission/Arrey Jiu-Jitsu and its trainers, instructors, volunteers, and all staff members harmless from any and all liability, loss, or damage, personal injury, or wrongful death caused or arising from their negligence, or those of others, including myself. 

 My signature on this document will also prevent my heirs, assigns, representatives, legal guardians, or any person who may sue on my behalf, from suing as well. I understand that by voluntarily participating in this activity, there are risks of physical injury to my person or property, as well as risks due to the negligent conduct of Riverside Submission/Arrey Jiu-Jitsu and its employees, myself, or others involved with the activity. Fees, times and dates of all events are subject to change. Please advise ALL participants involved in Riverside Submission/Arrey Jiu-Jitsu are subject to being photographed and videotaped, By signing the below, l acknowledge and declare that l understand the legal consequence of this release. I understand that any medical treatment for injuries sustained during participation in the Riverside Submission/Arrey Jiu-Jitsu program will be sought independently of Riverside Submission/Arrey Jiu-Jitsu. Riverside Submission/Arrey Jiu-Jitsu does not provide medical advise or services.
